What: Jacob’s Well BLOOD DRIVE with Memorial Blood Centers

Where: Bloodmobile at the Jacob's Well office (4751 Chicago Ave S)
How: Sign up online (FIREFOX users may have to scroll down).

Once you enter the sponsor code and click on the blood drive date that appears, you will be asked to create an account which will be used for scheduling this and any other future appointments. Please enter the personal information requested such as your name, telephone number, blood type (if known), etc. This information will be kept confidential and will not be used by Memorial Blood Centers for anything other than online scheduling purposes.
When complete, click on save and you will be brought to the schedule for the drive. Click on any available time to schedule your appointment. After clicking on "schedule appointment" you will first need to confirm the details and click "confirm appointment" before you appointment is scheduled. (Please don’t forget this 2nd step of clicking a second time to CONFIRM APPOINTMENT to be sure that your appointment is actually saved online.) You will then receive a confirmation e-mail containing the appointment details.
